Stan Smallwood, CRNP

Stan is a Board certified Family Nurse Practitioner who has 19 years as a FNP and along with 20 years combined experience as a RN and Respiratory Therapist. His areas of interest include adult primary care, diabetes, hypertension, and cardiac. When not working he enjoys being outdoors and watching sports.

Memberships For Uninsured Individuals and Families

Membership plans available for individuals or families starting at $79/month for a single adult; second adult is $39/ month; each child $19/ month with max $149/ month for family of four. $19 per additional child if you have more than two children. A $5 per person visit fee is also assessed at each visit. All individuals must live in the same household to qualify. Three month commitment is required. THIS IS NOT INSURANCE. Contract required.
